A case of treatment of a patient with blood test abnormalities and fever

A 52-year-old man was admitted to the hematological center with a suspicion of a hematological disease with complaints of severe general weakness, skin itching, an increase in body temperature to 38 0C in the evening, weight loss of 7-8 kg in 6 months, periodically pain in the left hypochondrium, mostly during physical exertion. He has been ill for about 1 month. The patient consulted a family doctor about 1 week ago. The patient is counseling in the hematology center after preliminary diagnosis and detection of changes in the complete blood count.

The purpose of the training – is to acquaint platform users with diagnostic standards, identify the risk groups, make the diagnosis and choose the treatment in case of failure of first-line therapy in patients of an unfavorable risk group.

  • Determination of typical complaints and symptoms of a certain hematological disease;
  • Physical examination of the patient;
  • Analysis of the patient’s outpatient card;
  • Appointment of the necessary lab tests and analysis of the results;
  • Appointment of the necessary diagnostic tests and analysis of the results;
  • Evaluation of myelogram;
  • Carrying out a differential diagnosis;
  • Making a clinical diagnosis;
  • Stratification of patients by risk group depending on cytogenetic and molecular changes;
  • Acquaintance with international nosology classifications;
  • Selection of 1 line of treatment depending on the risk group, age and physical condition of the patient;
  • Determination of response to treatment;
  • The choice of a further treatment strategy for this patient depending on the response to treatment in 1 line;
  • Choice of treatment for a patient with refractory disease.
