Calculation
High risk (>3 points). An anticoagulant is recommended if it has not been prescribed before:
- Enoxaparin 40 mg subcutaneously 1 time/day
- Dalteparin 5000 units 1 time/day (category 2B)
- Warfarin (INR 2-3)
Low risk (≤3 points). Recommended:
- Do not prescribe medications
- Aspirin 81-325 mg/day
