Current thromboprophylaxis: low molecular weight heparin (LMWH) at prophylactic doses or aspirin-3  
Current thromboprophylaxis: LMWH at prophylactic doses or warfarin-4  

Calculation

High risk (>3 points). An anticoagulant is recommended if it has not been prescribed before:

  • Enoxaparin 40 mg subcutaneously 1 time/day
  • Dalteparin 5000 units 1 time/day (category 2B)
  • Warfarin (INR 2-3)

Low risk (≤3 points). Recommended:

  • Do not prescribe medications
  • Aspirin 81-325 mg/day
