]]> ClinCaseQuest is proud to highlight the presentation of Tetiana Antofiichuk, MD, PhD, at the international scientific-practical conference “Medical Simulation: A Look into the Future”, held at Bukovinian State Medical University.

Dr. Antofiichuk is an educator at one of the validation centers of the ClinCaseQuest simulation training platform, where she teaches clinical reasoning – a core competency that our simulation scenarios are designed to develop.

Her presentation introduced an analytical approach developed within ClinCaseQuest for evaluating clinical reasoning in simulation training: Attempt-Based Analysis

Traditional simulation assessment typically measures only the final correctness of a clinical decision. However, this approach may conceal critical differences in the reasoning process that leads to that decision.

The study conducted using ClinCaseQuest simulation scenarios demonstrated that:

  • 100% of students reached the correct final diagnosis in both scenarios
  • however, their clinical reasoning pathways differed significantly
  • on average, students required 2.3 attempts per key clinical decision step to reach the correct outcome

By analyzing attempts, types of errors, self-correction patterns, and decision-making time, this method reveals how students think, rather than only what decision they ultimately make.

The research also identified five distinct clinical reasoning types, ranging from structured expert reasoning to trial-and-error decision making.

Even more importantly, the analysis uncovered four systemic learning zones where targeted educational interventions are needed:

  • Early severity assessment of hemodynamic instability
  • Errors in ACLS decision sequences
  • Over-testing driven by the “more tests = safer” mindset
  • Gaps in ECG interpretation despite theoretical knowledge

These findings demonstrate an important insight for medical education:

The correct answer is not the finish line – it is where meaningful analysis begins.

Attempt-based analysis complements traditional evaluation by making the clinical reasoning process visible, enabling educators to design more precise and effective teaching interventions.

ClinCaseQuest is grateful to Bukovinian State Medical University for serving as a validation center and supporting this research initiative.

We presented our strategy to teach doctors and teachers as experts for the Simulation Training Platform “ClinCaseQuest” as a strategy that can be used for medical school teachers as well as “The Program for training medical school teachers to write scenarios of clinical cases with branching”.

We are very grateful to the entire team of ClinCaseQuest experts, doctors, and teachers. Only thanks to our joint work our algorithms and drafts have been significantly improved, which became a powerful basis for the presented report, every week we analyzed the work on errors and looked for ways to optimize each weekday and night an important update. When registering in the Simulation Training Platform “ClinCaseQuest”, you accept the terms of service (public offer) – the user agreement, which discusses the following facts:

  • Training scenario for creating OSCE station simulators (Objective Structured Clinical Examination)

Thematic publications for the platform blog should contain links to sources used in the preparation of training materials and contain links in the text in accordance with generally accepted publications in scientific and practical sources.

If the training scenario of a clinical case is based on a real clinical situation with the visualization of additional research methods, then the information from the author to the “Global electronic database of clinical case simulation scenarios “ClinCaseQuest” occurs in a blinded form without indicating the patient’s personal data and after depersonalization of personal data in case of providing the patient’s consent to the transfer of information about his/her illness, its course to the “Global electronic database of clinical cases simulation scenarios “ClinCaseQuest”, which is confirmed by his/her familiarization with the information about the platform and the signing of the patient’s informed consent.

If a group of authors is involved in the preparation of training materials, then the copyright applies to the group of authors.

Authors collaborating with the Simulation Training Platform “ClinCaseQuest” are encouraged to complete the registration procedure on the platform, add their profile photo and profile cover, and fill in personal information in three languages ​​regarding:

An author’s agreement is signed between the authors and the administration of the Simulation Training Platform, according to which the authors receive royalties for the work done on the preparation of training materials and the transfer of copyright property rights from the author to the administration of the platform, as a result of which, the exclusive right to use the created training materials will belong to the platform administration.

When authors prepare materials for a professional school, authors, with the consent of the parties, can receive a quota percentage of sales from the platform marketplace.

The Simulation Training Platform retains the copyright right to compile learning materials on the platform.

4. Copyrights of third parties

When does blocking happen?

Blocking occurs when you try to enter the simulation learning platform and enter your password or login incorrectly several times consistently (usually at least 5).

    The number of “quest rooms”, the so-called simulation locations in the structure of a clinical situation may be different, and is determined primarily by the logic of the clinical situation in real clinical practice, the complexity of the issues involved in solving the clinical situation, of clinical resources involved in solving the clinical situation, specialists, etc.

    Only when you perform all the tasks correctly – the passage of the simulation location will be counted. And when you pass all the locations in each clinical situation, you will be credited with a whole simulation training. The progress of the simulation locations is saved. And you can go through different simulation locations on different days, because sometimes the passage of such quest scenarios can take from 3 to 6 hours.

You can access the Platform’s news feed either from the shortcut menu by clicking on the “News Feed” icon, or by using the vertical drop-down menu by selecting the “News Feed” tab. The “News Feed” page in the network of the “ClinCaseQuest” platform will open.

    The news feed is an integral part of communication on the Platform, obtaining general information about all updates, notifications about new and current trainings, creation of new simulators of clinical cases by the Platform, and exchange of experience between doctors, acquaintances, establishing contacts, discussing medical information, etc.

    The news feed of the Platform provides information on site updates, public announcements of users, creation of new discussions and debates in medical communities, trainings, discussion of clinical cases. With the increasing of the number of users and, consequently, the number of discussions, you will receive more relevant content in the news feed, that is content that may be of interest to you, related to the training in which you study, the medical communities in which you are registered, your messages and discussions, your subscriptions, your friends, etc.

    To place a post in the Platform news feed, start composing a new message in the empty block that goes first in the news feed. You can insert text or print it.

    You can also format text by clicking on the “Aa” icon:

    • to make it more noticeable, highlight; 
    • to make the text italic;
    • to create an unnumbered list;
    • to create a numbered list;
    • to add citations;
    • to add hyperlinks to post;
    • to provide a pre-formatted text

    Once the text has been edited, you can press the icon “Aa” again to return to editing the message.

    In addition, you can add to your post:

    • images;
    • videos;
    • attach a document;
    • and add emotions.

    When the publication is ready, please select the placement and display options of your publication.

    You can publish the publication in your profile or in the group (medical community) in which you are registered.

    If you choose placement options in your profile, you can then choose who will see this post.

    Your post will be visible to:

    • all registered on the Platform users – these are “Public” and “All Members”;
    • only your contacts, that is, those registered users whom you have connected with (i.e. your friends);
    • or only you – and this will be a private post in the news feed of the Platform.

    In order to select “Publish to group” (medical community), you must be a member of one or another medical community.

    In the next field, start entering the name of the medical community to place your post, an image and community name will appear, select the community you want to post your post by clicking on it. Once you have selected the desired mode for posting and displaying your message in the Platform news feed, click “Post Updates.”

    Or, if something fails, you can click “Cancel” and start creating the post all over again. If you select “Post updates”, your post will appear in the Platform’s news feed, in your profile when you add the post to your profile, and in the relevant medical community when you add the post to the medical community.

     If you have selected “Public” and “All Members”, your post will appear in the news feed to all registered users, mainly to your friends.

    You can edit or delete your posts in the news feed.

    You can also add comments to your post at any time:  

    • print comment text;
    • add images, videos;
    • attach documents;
    • add emotions.

    To save a comment, click “Record”, or to not save it, click “Cancel”. Like and add comments to the posts.

    According to the privacy policy of the Global Electronic Database of Clinical Case Scenarios “ClinCaseQuest”, user profiles are available for viewing only to users registered on the Platform and are not available for indexing in search engines, i.e. are not public to the Internet users. Only users registered on the Platform can view each other’s profiles.

    However, you can decide for yourself which fields of your profile will be available on the internal network of Platform for viewing and which will not. That is, whether all users will be able to see your personal data, data about your education and professional activity.

    To change the privacy settings of your profile fields, first go to your own profile using the drop-down menu and select “Edit profile”.

    In the open browser window you will see several lines with your data: personal data, education, professional activities.

    Select the desired parameter and change the settings. To change the publicity settings of a field, click “Change” and select who will be allowed to see this field.

    The “Public” and “All members” settings mean that all users registered on the Platform will see this information, “My contacts” – only those users whom you have added to your contacts, “Only me” – only you.

    Select the parameter you want to publicize and click “Close”.

    Don’t forget to save the changes.

    You can go to other tabs and select the publicity settings for each field.

    You can change it by editing your profile – clicking the tab with your image and name in the drop-down menu and then – clicking “Edit” and selecting the appropriate parameter:

    • personal data;
    • education;
    • professional occupation.

    Click “Edit” to select the availability of each field. We’ve covered this in detail in the Profile Fields Publicity Settings guide.

    Now we’ll discuss another way to change profile visibility settings.

    In the top drop-down menu, click the “Edit Account” tab. On the settings page, select the “Privacy” tab on the sidebar.

    The “Profile Visibility Settings” page opens. Here you can make mass settings of all parameters:

    – visibility “Public” and “All members” – information will be available to all registered users of the Platform;
    – “My contacts” – the information will be available only to colleagues with whom you have connected in the Platform;
    – “Only me” – the information will be available only to you.This way you can change the display settings:

    • of your personal data:
      • first name, last name, nickname;
      • social media profiles;
      • additional information about you
    • education information:
      • higher education institution where you studied;
      • degree;
      • academic title;
      • additional information.

    After selecting the field visibility options, click “Save Changes”. In addition, you can change the settings of the “Professional Activity” section by selecting the required parameter to change the display publicity settings:

    • year of onset of medical activity;
    • speciality;
    • qualification category;
    • place of work;
    • additional information.

    You will be taken to the “Professional Occupation” profile page.

    Click “Edit” to check the privacy settings of the fields in this section. Select the required field, click “Edit” and select the visibility option (“Public”, “All members“, “My contacts” or “Only me“). After selecting the field visibility settings, click “Save Changes“. Thus, you have chosen convenient profile visibility parameters.
